The Center for the Advancement of Innovative Health Practices has created the Cycle of Engagement model and tools for use by clinicians, providers, practices and organizations to assess and improve the quality of child care and promote child and family health. This website was developed and is maintained by the Center for the Advancement of Innovative Health Practices which is a non-profit arm of the Child and Adolescent Health Measurement Initiative (CAHMI).
The following Terms and Conditions govern your use of the Cycle of Engagement website. This website is made available to you by the Center for the Advancement of Innovative Health Practices. We may change the Terms and Conditions from time to time, without notice to you, by posting the updated Terms and Conditions on the website. We will use reasonable efforts to identify the changes in the Terms and Conditions. BY USING THE WEBSITE, YOU ACCEPT AND AGREE TO THESE TERMS AND CONDITIONS AS APPLIED TO YOUR USE OF THE WEBSITE AND AS MAY BE AMENDED. If you do not agree to these Terms and Conditions, you may not access or otherwise use the website. By using the website following any modifications to these Terms and Conditions, you agree to be bound by any such modifications to these Terms and Conditions.
The Cycle of Engagement is comprised of the Well-Visit Planner (WVP), the Online Promoting Healthy Development Survey (Online PHDS) and the Care Path for Kids (CPK). By registering on the COE website, you get access to the COE dashboard from where you can enable to use and access the WVP and Online PHDS Portals. You also get access to the CPK website which has the CPK tools and implementation resources. You can use the WVP and Online PHDS Portals to:
- Customize the Well-Visit Planner (WVP) with a unique URL, logo, additional assessment and screening tools, and resources
- Customize the Online Promoting Healthy Development Survey (Online PHDS) with a unique URL, logo and resources
- Track the WVP and Online PHDS usage by parents
- Generate summary reports from multiple parents’ responses on the Online PHDS specific to your practice or organization.
- Get access to educational, implementation and family engagement resources
- Obtain Maintenance of Certification (MOC, Part 4) credit from American Board of Pediatrics (ABP) and Continued Medical Education (CME) (coming soon on the Online PHDS Portal).
By using this website, you expressly agree not to abuse it. Children's Online Privacy Protection Act
We are in compliance with the requirements of COPPA (Children's Online Privacy Protection Act). We do not collect any information from anyone under 13 years of age. Our website, products and services are all directed to people who are at least 13 years old or older.
Proprietary Rights, including Copyright and Trademarks/Service Marks
The Center for the Advancement of Innovative Health Practices owns and retains all proprietary rights in the Cycle of Engagement website. All content, formats, information, and functionalities of this website are copyrighted by the Center for the Advancement of Innovative Health Practices. The Center for the Advancement of Innovative Health Practices shall retain exclusive right, title and interest (including all intellectual property rights) in and to all materials that are part of the website (including past, present and future versions), including, without limitation: art, icons, graphics, layout, text, images, audio and/or video, designs, features, advertisements, logos, domain names, trade names and marks, and service marks, any and all copyrightable material, the “look and feel”, the compilation, assembly and arrangement of the materials of the Service, and all other materials or content made available in the website and any such website content is protected from unauthorized use, copying and dissemination by applicable copyright, trademark, patent, publicity and other laws, rules, regulations and international treaties.
The information gathered from you on the Cycle of Engagement website is confidential. We do not save any personal health information and we do not share the patient individual information with you as the clinician or health care provider. Clinicians, health care providers and health systems that have an account with the COE and have enabled to use the WVP Portal can request for a summary report of the aggregated data collected on the WVP parent tool. Clinicians, health care providers and health systems who have an account with the COE and have enabled to use the Online PHDS Portal can generate summary reports of aggregated findings based on parents’ responses on the Online PHDS parent tool, specific to their practice or organization. Clinicians and health care providers will not know what the parents’ individual responses are or be able to identify who completed the Online PHDS. The Center for the Advancement of Innovative Health Practices team can use the aggregated findings from parent responses on the WVP and Online PHDS tools for research purposes.
Account, fees, billing methods and termination
(a) Accounts. By completing the registration process, you may establish a user account (“Account”) and become a registered member (“User”) and access the website. To create an Account, you must provide such information as may be requested during the registration process or requested from time to time in our sole discretion. You must provide truthful and accurate information during the registration process. You will receive email and other communications from us relating to or deriving from your use of the website. By disclosing your email and other contact information during the registration process, you agree that we may send you such communications. You will no longer receive communications contemplated by these Terms once your Account is terminated or you opt-out of receiving such communications.
You are solely responsible for all activity on or related to your Account, your customized content, and for the security of your computer, mobile device, and any other device that you use to access the website. You should not reveal your Account password to other users or permit others to access your Account. All acts performed using your Account shall be deemed to be your acts. However, the Center for the Advancement of Innovative Health Practices, in its sole and absolute discretion, may, from time to time, modify, amend, or supplement the terms and billing methods relating to fees and charges, and post those changes on the website. Such modifications, amendments or supplements shall be effective immediately upon posting on the website. If any change is unacceptable to you, your sole remedy is to cancel your Account and cease using the website and/or services.
